Do I need to move files into separate folders to organise them?

No. Ratings, colour labels and filters let you organise the view while keeping the files in their existing folders.

Why might a search miss a photo I can see in a folder?

Search becomes more complete as Lighthouse indexes files and analyses available previews. Recognised subjects and text depend on that analysis and may not be available for every image.
